New document required for an ICHSC licence
Establish Ownership of CSDC and complete a Certificate of Status from the Ontario Business Registry, dated as within 15 days of the date of submission to the ICHSC Program, Ontario Ministry of Health.
Requisition forms for ICHSCs
Centres are requested to advise patients that requisition forms can be used to access health care services in a centre of the patient’s choice.
